This is a fixed-term role through to 30 June 2026, primarily based in Horowhenua.About the Position:As a Health Coach, you will work collaboratively with whanau whaiora and the THINK Hauora team to promote self-management skills and provide support tailored to the needs and aspirations of whanau.
You will liaise with various mental health and addiction services alongside primary health care teams, offer holistic monitoring of wellbeing needs, and coordinate access to the best support options available.Supporting the health and wellbeing of New Zealanders is changing and evolving for the better.
Emphasis is placed on reducing barriers to accessing services and embedding a 'skills not pills' approach within primary care that empowers people to lead their own physical, mental, social, spiritual wellbeing.THINK Hauora is keen to hear from applicants interested in a career path as a Health Coach within primary care.
This role offers you a chance to positively impact the lifestyle and wellbeing of a range of people enrolled in general practice settings.
You will be working in partnership with patients and whanau, GPs and nurses to help patients gain the knowledge, skills and self-confidence to reach their health goals.
You will be empowering people with their lifestyle changes (smoking cessation, exercise), promoting self-management (especially for managing long-term conditions like diabetes) and helping navigate primary care and the wider social services they need to help them succeed.Working as a Health Coach offers a busy but incredibly rewarding environment, whereby you will tangibly get to see the difference and positive impacts that you can have on people's lives.Key responsibilities and competencies required to undertake the role include (but are not limited to):Proactive relationship management with a diverse range of people in a culturally appropriate manner.
You will build and maintain positive relationships with whanau whaiora with their health needs and/or disabilities.
You will also identify and create opportunities for whanau whaiora to connect with service providers and stakeholders.Bridging health barriers to support, guide and navigate persons with health needs and/or disabilities within the population to connect with service providers in order to improve their health outcomes.Improving health outcomes for the community by assisting whanau whaiora to enhance and develop their own skills and knowledge to build on strengths and confidence.Networking by way of building and maintaining relationships that will assist in providing opportunities for whanau whaioraWe are looking for someone who has the ability to work in a fast-paced role, a self-starter, capable of managing their own workload, excellent communication skills and a positive can-do attitude.
You will be committed to ensuring best practice when working with whanau whaiora.
You will find solutions that best meets their wellbeing needs and will have the ability to communicate with a diverse range of people.
